Rights and Procedures to File a Complaint
If you feel you are at any time treated in an unethical manner by therapist's
name as a client at agency/practice name, it is your right to file
a complaint with the Board of Social Work Examiners and/or the Director of the
Department of Labor, Licensing, and Regulation. You may file a complaint by sending
a letter directly to the Board or call the Board for a form to fill out and return.
